Register or Request InformationAt IFM, we care deeply about the health and safety of children, teens, and young adults in our community. Some heart conditions in young athletes may not cause obvious symptoms, and in rare cases, hidden heart problems can lead to serious cardiac events during sports, exercise, summer camps, practices, games, or other physical activity.
Our goal is not to create fear. Our goal is to help families feel informed, supported, and proactive about their child’s heart health.

Most young athletes are healthy. However, some heart conditions do not cause obvious symptoms. In rare cases, hidden heart problems can lead to serious or even fatal cardiac events during sports or physical activity.
While no screening can detect every possible condition, early screening may help identify concerns that need further attention. For many families, this type of screening provides reassurance, peace of mind, and an opportunity to be proactive.

This screening is a very helpful tool, but it should not be considered a diagnostic test. A normal result does not guarantee that there is no heart problem. An abnormal result does not always mean there is a serious condition. Some participants may still need follow-up care with their pediatrician, primary care physician, or cardiologist.

At the time of screening, we collect the athlete or participant’s full name, date of birth, age, gender, school name, grade, sport(s) played, date of screening, address, phone number, and email address.
For minors, we also collect the parent or guardian’s full name, relationship to the participant, phone number, email, and address, along with an emergency contact name, relationship, and phone numbers.
Before screening, we ask a short set of yes or no questions:
If the answer is yes to any of the above, we ask for a brief explanation.
Participant and parent understand that the Youth Athlete Cardiac Screening offered by IFM is limited to a Cardiac Wellness Screening / ECG, a cardiologist-reviewed report, and a brief discussion of results with a recommendation for follow-up with the participant’s physician if needed.
Participant and parent understand that this screening is limited in scope and is not intended to replace a comprehensive medical evaluation by a pediatrician, primary care physician, cardiologist, sports medicine physician, urgent care center, or emergency department.
Participant and parent understand that this is a screening only. Screening results are not diagnostic. Screening does not guarantee the absence of heart disease, cardiac abnormalities, arrhythmias, or future cardiac events.
A normal screening result does not guarantee that the participant is free of cardiac risk. An abnormal or concerning result does not confirm a diagnosis. Further evaluation by the participant’s pediatrician, primary care physician, or specialist may be necessary.
Participant and parent understand that the screening is intended only as an optional wellness tool to help identify possible areas of concern that may require further medical attention.
Participant and parent understand that IFM is not responsible for sending screening results to the participant’s pediatrician, primary care physician, cardiologist, school, coach, or sports organization.
Participant and parent are responsible for sharing screening results with the participant’s physician if desired or recommended. Any further evaluation, diagnostic testing, sports clearance, treatment, or referral is their responsibility.
By signing, the participant or parent confirms and understands that:
Participant and parent understand there are inherent limitations to any screening process and voluntarily assume any risks associated with participation in the screening.
Participant and parent agree not to hold IFM, Dr. St. Claire Sumaili, employees, contractors, assistants, collaborating physicians, or affiliated personnel liable for undetected conditions, false positive or false negative findings, delays in follow-up care, decisions made by the participant, parent, coach, school, or physician after screening, or future cardiac events, injuries, illness, disability, or death.
Participant and parent release and waive claims against IFM and Dr. St. Claire Sumaili except in cases of gross negligence or intentional misconduct.
With consent, participants may be contacted by IFM regarding screening results, recommendations, future wellness events, or related services.
By signing at the time of screening, the adult participant or parent/guardian acknowledges that they have read, understood, and agreed to all sections of this Consent, Authorization & Liability Waiver. Signature lines are provided for the adult participant, the parent or guardian of a minor (with printed name and relationship), and an IFM representative.
